Application Form for Graduate Certificate in Global Surgical Care

Your application to the Graduate Certificate Program is sent directly to the Branch for International Surgical Care Office (see details below). Please ensure you have followed the GCGSC Application process before submitting your application.

When completing the “Course Goals” section of the application, for each of the courses, please indicate in what year of your program you intend to register for each course. If you are currently working and have other commitments we recommend taking one course per semester as each course will require between 6 to 8 hours of your time per week.

This can be done by email or by fax 604 875 4036 to the attention of the Manager, Branch for International Surgical Care.

If sent by fax, please email the Manager to confirm that your application has been received.
